Careflex; Mediflex forms due to GGA
All Flexible Spending Account participants must mail 2002 Mediflex and Careflex claims to Third Party Administrator, Glynn Griffing & Associates, P.O. Box 16509, Jackson, MS 39236-6509, on or before March 1.
